Accreditation of personal autonomy promotion services in Castilla y León

Purpose:

To establish the requirements that must be met and the administrative procedures that must be carried out so that certain services promoting personal autonomy for people in situations of dependency, provided or managed by public or private entities, obtain the accreditation that guarantees that said services meet the requirements and quality standards that are demanded of them.

WHO CAN APPLY FOR IT?

  • Public or private entities, whether natural or legal persons, for-profit or non-profit, that provide or manage the following services for the promotion of personal autonomy:
  • a) Rehabilitation and occupational therapy service
  • b) Cognitive stimulation service
  • c) Functional promotion, maintenance, and recovery service
  • d) Psychosocial habilitation service for people with mental illness or intellectual disability

What documents do I need?

For accreditation, an application must be submitted using the standard form, accompanied by the following documentation:

  • Internal Regulations.
  • Service Charter.
  • General Service Program.
  • Individual Care Program Template.
  • Service User Contract Template.
  • Organizational Chart of the Service Provider.
  • Civil Liability Insurance Policy.
  • Complaints and Suggestions Form.
  • Information regarding accessibility for people with disabilities at the center or service.
  • Declaration confirming compliance with tax obligations (according to the application template).
  • Declaration confirming compliance with the quota for employees with disabilities established in the consolidated text of the General Law on the Rights of Persons with Disabilities and their Social Inclusion, approved by Royal Legislative Decree 1/2013, of November 29.

For accreditation renewal, an application, using the standard form, must be submitted along with the following documentation:

  • A report on the accredited service's activities, with separate sections for each of the four years covered by the accreditation.
  • A declaration confirming continued compliance with the quality criteria and standards required for obtaining accreditation, as per Article 10.3 of Decree 3/2016, of February 4.

WHAT TIME DO I HAVE TO ASK?

In general, there is no deadline for requesting accreditation of the services listed; the entity providing or managing them may request accreditation when it meets the requirements. However, entities that, as of the date of publication of Order FAM/2/2015, of January 7, were providing the services listed, financed with public funds, including services linked to the service, will have a maximum period of one year to request accreditation, starting from the day after the publication of said Order.

DEADLINE FOR RESOLUTION:

  • 45 days from the date the application is entered in the register of the competent body to resolve.

NOTIFICATIONS:

  • By Postal Mail at the address of the interested party.
  • Electronic notification by appearance, by means of a notice via the e-mail address included in the application. This notice shall not have the effect of a notification until the interested party accesses the content of the notification in the NOTI notification system.

ADDITIONAL INFORMATION:

In accordance with the provisions of Articles 14.3, 16 and 17 of Law 39/2006, of December 14, on the promotion of personal autonomy and care for people in situations of dependency, accreditation must be obtained in all cases so that the services for the promotion of personal autonomy can be financed with public funds, including the benefit linked to the service.

  1. Provide an  electronic certificate and  technical requirements.
  2. Access the processing, being able to make use of three possible different means, where appropriate::
    1. Through an Application: Authenticate (electronic certificate, DNIe, Cl@veFirma, Educa account, etc.), select the procedure, fill in the required fields and follow the instructions on the screen..
    2. By means of an Online PDF Form  ( Instructions for carrying out electronic procedures ) by opening the PDF directly with Adobe Acrobat Reader or saving it, filling in the required fields and sending through  Electronic Administration Application
    3. Via Web Form: Fill in the required fields for each section, click "Review" and make the appropriate corrections, if applicable, and then click on the electronic processing option ("Submit/Send/Save")..
